Many of these forms are provided for your convenience to help you prepare before coming into our branches and program locations and to help improve your experience with the YMCA. It is important to understand that these forms may not apply to every situation and in many cases there may be additional forms to register for a program. If you find that this site is does not have a form that you think would be useful to provide in this section in the future, please contact your local YMCA branch.

Financial Assistance
- DeGraff Scholarship Application 146 KB
- The Harmon O. DeGraff Memorial Scholarship Fund was created to assist young men with a strong human relations orientation to pursue graduate study. (For males only by terms of the Will.) This .PDF form contains information regarding this scholarship fund and application information.

- Camper Information Form 91 KB
- This form contains information that will be shared with the camper's counselor with specific details about the camper's emergency contact info, skill and abilities, and personal likes and dislikes.

- If your camper needs to leave camp before the end of the normal camp week it is essential that we know this in advance. This form can be sumbitted during check-in to improve our ability to make sure your camper is ready to go when you come to pick them up.

- This Health Form should be completed by every summer camp participant prior to attending camp. A portion of this document should be completed by a physician every two years as part of a regular exam.

- As with most camps, the fees that we charge do not cover the cost of major improvements to the facilities and grounds. If not for the donations of individuals, businesses, and foundations, we would not be able to build new and renovate existing buildings to expand the quality of our programs. This list was developed by the Camp Executive Board and Staff outlining the most pressing needs for camp to continue to operate successfully.
